Note that incident date, title, category (Occupational Safety, Fire Safety, Chemical Safety, Machine Safety etc.), type (Positive / Constructive Observation, Near Miss, Unsafe Condition, Unsafe Action) and priority (Critical, High, Medium, Low) will be fixed fields in the incident form and not part of the template questions, as they are essential for all incidents regardless of the specific details.
